To register and pay, you must repeat each step below for each player you wish to register. For multi-player families, we will still honour our discount policy, so please email us a note to request your discount at info@fccapitalunited.com. You may use the same email address if you have questions.

- Proceed to the Checkout page to pay the fees for that player, or send your cheque to our mailing address (see Contact Us for mailing address).

- I understand that I or my child/ward cannot play in any sanctioned soccer game until after this registration form has been validated and the registration data has been entered in The Ontario Soccer Association's computerized registration system.
- I have reviewed the waiver/participation agreement (for players 18 and older or for players under age 18) and indicate my agreement with such waiver/participation agreement.
- I am aware of The Ontario Soccer Association, Eastern Ontario District Soccer Association, Capital United Soccer Club and League bylaws, policies, rules and regulations and agree to abide by them and to be bound by them.
- I accept sole responsibility for my or my child/ward’s personal possessions and athletic equipment.
- I accept all liability for any damage to the playing equipment caused by me or my child/ward’s careless, negligent and/or improper handling.
- I accept the terms of the Club's refund policy and agree to abide by the decisions of the Board on these matters.
